California State University (CSU) Approved Health Insurance
(Recommended by San Francisco State University and other CSU schools)
This is the health insurance brokered by Wells Fargo bank. Wells Fargo of California Insurance Services, Inc., (https://wfis.wellsfargo.com), is a very well known corporation in the US. A health insurance brokered by this bank would definitely be a quality insurance policy. The actual insurance provider is Anthem Blue Cross Life and Health Insurance Company. Once again, this is a very well known company in the US. An insurance policy that is underwritten by this company would actually be of a better quality than others.
Details about the insurance can be found at this link:

The cost per semester is approximately $630.
The insurance covers Doctors’ fees, Hospital fees, and Surgery fees. It DOES NOT cover Ambulance fees and Medical fees. If you want to cover these two too, then you might have to take additional insurance (a little more every month, or per semester).Personal recommendation of students is that these are not needed.
Ambulance Fees
Ambulance fees are covered by the Blue Cross insurance when it is an emergency. It is only when you use the Ambulances when it is NOT an emergency that you get charged for that. It is always better to call the insurance agent and verify what the company actually means by an “emergency”.
Medical Fees
As for the Medical fees, medicines will cost almost the same or slightly more than what you have to pay the company for the extra insurance, unless, of course, you require several medications. Thus, unless you feel that it is absolutely necessary, it is recommended to not take any extra insurance for the prescription.
For questions regarding coverage, benefits, etc., call Anthem Blue Cross at 1-800-888-2108.
American Insurance v/s Indian Insurance
Insurance from India is cheap, but claiming procedures are complicated and time consuming. Eventually you will end up paying a huge amount up front, follow up for claiming procedures, and finally you will get paid at your Indian address. So you will be at loss in terms of time, money, and dollar exchange value.
US insurance is expensive, but valid all over and does not require any money over the counter. The student needs to pay a small fee called "Co-Pay" at the time of treatment (or sometimes after treatment) and present the Insurance card. The Co-Pay is usually in the range of $10 to $50 and is different for every region, insurance company, and type of treatment.
